Forms
You must be a Brooklyn College faculty member to access the following forms in the Brooklyn College WebCentral portal. Once you log in to the portal, select "Research Compliance—IACUC" under the Academics tab.
- Animal Use Protocol Application (initial and three-year renewal submissions)
- Annual Renewal Form (continued IACUC approval during years one and two of project)
- Amendment Request Form (request for change to approved protocol)
- Personnel Change (adding and removing personnel from an approved project)
