Personal Data Change Requests
Due to the COVID-19 pandemic, we are currently unavailable to answer calls and accept documentation in person for changes to personal information. The college is providing services remotely at this time.
You can request to change personal information such as name, address, telephone number, and Social Security number in Brooklyn College records by submitting the Personal Data Change Request Form (pdf) along with required documentation via e-mail.
Some personal information items require documentation to be submitted with the form:
- Documented Name Change / Correction of Documented Name—Photo ID and one of the following: marriage certificate, passport, birth certificate, Social Security card, divorce decree, or court order. Note that middle names are not displayed in CUNYfirst and do not appear on official printed documents.
- Date of Birth Correction—Photo ID and the original (or official replacement/duplicate) birth certificate.
- Social Security Number Change / Correction—Photo ID and the originally issued (or official replacement/duplicate) Social Security card.
- Permanent Address Change / Correction—Utility bills, other formal postal mail, a New York State–issued driver's license or a non-driver's ID. Changes to other address types (home, mailing, billing) do not require documentation and can be accomplished in Self-Service in CUNYfirst.
Changes to personal information such as e-mail addresses, gender, home/mailing/billing addresses, and phone numbers can be accomplished in your CUNYfirst account. Under the Personal Information section of your Student Center, select the information you would like to update and click "save" when you are finished.
